Paediatric advance care (PAC) Plans
The Paediatric advanced care plan has been designed to be used across the principality to help guide and document decision making with families.
The PAC plan can be used with families of children and young people with life limiting (LLC) or life threatening conditions (LTC). It is hoped that all services supporting children and families with LLC or LTC will use it when supporting families with difficult decisions to ensure consistency for families and good communication.
The PAC plan, process and supporting guidance / education was last updated in 2021.
