The palliative care network encompasses and works closely with the Children’s’ Hospices , but they are all independently run and funded and are not part of the NHS. They are funded by charities.

Ty Hafan is based in Sully, South Wales. It is mainly run by nursing staff with medical support from Dr Megumi Baba, consultant in paediatric palliative care and a team of local GPs who have contracted hours to the hospice.  Ty Hafan also provides office space for the rest of the Cardiff and Vale palliative care team and consultants, so they are often on site to provide additional support. Hospices accept the following for on-going care. Ty Hafan currently offers the following services:

 

• End of life care

• Compassionate extubation

• Care of a family where their baby has already died in order to support them spending time together with use of a cold cot

• On-going counselling and support services for families including siblings and wider family members such as grandparents, both during a child’s life and after death.

• Respite care during periods of stability.

• Bereavement support

 

Initial referrals can be made by telephone and the team will then inform you of the referral process and whether they are able to visit the family to discuss further . The pathway “South Wales Neonatal palliative care transfer pathway: NNU to hospice outlines what is required in terms of discharge documentation but it is sensible to check with the accepting team as it may vary depending on the baby’s needs.

 

There is a fine balance between exploring wishes with families and not offering hospice care to families prior to contacting a hospice to see if transfer is possible in order to provide realistic expectations.  If in doubt, discuss with the hospice team or your local palliative care team.
